On-Line Registration Form Instructions

Important:   Please Read.  If you do not understand, please do NOT complete the On-Line Registration Form.  Please send an E-Mail message with your question(s) to Timothy Arnold.

These statement concern Internet Privacy.  Any information sent through this form will be sent over an OPEN channel and could be intercepted by any one.  This information is not encrypted.  If you are concerned with Internet Privacy, carefully consider and weigh the advantages/disadvantages of completing this form or completing a paper copy and mailing it.  Students, please do NOT complete this form without informing your parents first.  None of this information is ever sold or given away.  It is used strictly for the Fair Registrar's purposes.   Address information is required only for winning entries.  If you have ANY questions, please contact one of our board members or Timothy Arnold.

These instructions are here to assist in the completion of the On-Line Registration Form.

There are certain fields which are absolutely required.  The required fields are indicated with a bold, red asterisk (*).   If any of these fields are left blank, you will be prompted to complete the field(s) missing data.

Project Advisor The name of an Adult who advised you on your project No
Is Electricity Needed Do you require electricity for your project display.  The default is NO. Yes
Category Select the Category in which you should be judged.  Be sure to select the correct Category in your Division. Yes

Middle Initial Self Explanatory. No
Last Name Self Explanatory. Yes
Mailing Address Street or P.O. Address or other No
Telephone (with Area Code) Enter your telephone number with the correct Area Code. No
City Self Explanatory. No

Zip Code Self Explanatory. No
Grade What Grade are you in?  Needed for correct Division information Yes
E-Mail Address If you are filling this out the On Line Registration Form, it is assumed you have an e-mail address.  This will be used to send you information about the Science Fair (confirmation notice, directions, etc.).  Also, it can be used if the Fair Registrar needs to contact you with any problems or questions. Student 2 and 3 information This is only required if a Team/Group project is selected.    Only the First and Last names of these students are required Conditionally

When you have completed the form, press the Submit button to send the information or click the Clear All Fields to reset the form.  If you click the Submit button, your data will be checked against certain criteria for validity.  If this check discovers any problems, you will be prompted with the problem and you will be returned to the form.
